AFFIDAVIT

That I, SHAHANIE L. ABBAT, of legal age, married, Filipino Citizen and lives at Landingan, Nagtipunan,
Quirino, after having duly sworn to min accordance with law do hereby depose and say the following to with
that:

1. I am a Teacher I at Wasid Integrated School, Nagtipunan, Quirino and I finished Master of Arts in
Education at Northeastern College, Santiago City on October 2023.
2. However, I was unintentionally, delayed with the submission of my Thesis Book for requirements of
application for issuance of Special Order for Graduation, due to financial constraints.
3. I, attest to the truth of the foregoing statements to the best of my own personal knowledge and based
on authentic documents and records. I am executing this affidavit for all other legal intents and
purposes it may serve.
