AFFIDAVIT

I, GRACE ROMA ERILLA, of legal age, Filipino and with residence at Bayanan, San
Vicente, City of San Pedro, Laguna, after having been duly sworn on oath in accordance with
law, do hereby depose and state:

1. That I am the owner/general manager of a business namely GRACE R.E

ENTERPRISES, located at San Vicente, San Pedro, Laguna (now City of San Pedro.)
Attached is the Department of Trade and Industry (DTI) Certificate and City Permit of
the said business for further reference;

2. That pertaining to the foregoing, as based in its New DTI Certificate issued on September
18, 2014 and with Certificate No. 03337582, said business is operating in its Barangay
jurisdiction, which is in Barangay San Vicente, City of San Pedro, Laguna;

3. That the nature of its business was enterprises, and more on services, particularly in
sewing;

4. Furthermore, the said business was operating more or less, eight (8) years and during that
time, it was operating in a micro-business in nature and scope;

5. That I executed this affidavit in order to attest to the truth of the foregoing facts and for
legal purposes it may serve most.

IN WITNESS WHEREOF, I have here unto set my hand this 2nd day of November, 2016 at
the City of San Pedro, Laguna.
