Absenteeism Among Secondary Students of Cotabato City Institute


The information provided below is to help you determine if you wish to
participate in the study. Hence, you are free to decide whether you do not want
to participate or withdraw without affecting you relationship with the researchers.
This will serve as Carla Villarba, Julia Recimilla, and Jaber Zailon’s research.

Purpose:

Teasley (2004) define absenteeism as a period of time when a student does not
attend school, has become major and continuous problem among high school
students in many countries. Many studies conducted to answer a question that is
why high school students miss classes. This idea, Teasley have noted multiple
risk factors that contribute to student absenteeism such as family health, low
income, poor school climate, drug and alcohol use, transportation problems, and
community attitudes towards education. Wilkins (2008) has reported four
themes, which play important role to motivate students to attend school such as
school climate, academic environment, discipline, and relationships with
teachers. Therefore, this study aims to determine the causes of absenteeism
among secondary students of Cotabato City Institute.
Participation:
Participation in this study involves answering of questionnaire, participation
in individual interviews, and brief follow up questions from the researchers. After
you have provided consent by signing this form, answered the backround
questionnaire and the researchers has received both materials, selected
participants will be contacted whether through e-mail or phone to answer
additional questions and to arrange the time to conduct the in-depth interview,
individuals who will not meet the criteria for participation will be informed of their
exclusion also through e-mail or phone according to preference. It is expected
that the participants will be included for approximately, one week and interviews
for a period of one whole week.

Confidentiality
In order to ensure the confidentiality of the participants, only two of the
researchers will meet and know the identity of the subjects.Each subject will be
given an alias or pseudonym to hide their true identity. All paperworks that have
been and will be used will be kept away securely and only the researchers
involved with the interview will have access to.All direct quotes that will be used
will be shown to the subject to ensure that the subject is comfortable with the
wording and will give them the freedom to change the statement to provide
sufficient anonymity.
Each participant will be given aliases to prevent their identification from being
discovered in the reports and results of the study.The participant’s real names
will be kept securely by the primary researcher locked away in a file cabinet.
Audio recordings of the interviews will be erased once the study is done and the
written works involved with will be disintegrated after atleast a year after the
closing of the study

You are encouraged to keep a copy of this consent letter, for it identifies your
consent to participate in this research study. If you have concerns or questions
to for the researchers, please contact Carla Villarba at

this consent, your answer will be valuable. Your signature indicates your
