Alkhidmat Mobile Health Unit

1.0 Introduction:
Alkhidmat is utilizing Mobile Health Units (MHUs) in response to the needs of underserved
communities in the flood-affected areas and to provide access to healthcare for millions of
people at their doorsteps.

MHU is a custom vehicle loaded with equipment, medical supplies, and skilled professionals, all
set to bring healthcare, which is otherwise inaccessible, closer to where people live and work.
The main reason for mobile health care is the provision of accessible services that are welcoming
to homeless people who can’t or won’t go to fixed-site clinics. Mobile units are the sole source
of health care in some rural communities and have also demonstrated their value in areas
devastated by floods.

1.2 Objectives:
I. To harmonize the operationalization of Mobile Medical Services in locations of Internally
Displaced People (IDPs) in camps and out of camps besides flood-impacted areas.
II. Maximize the utilization of health care services for the best interest of the flood-affected
communities
III. To facilitate the “Safe Mother Safe Family” Project in flood-affected areas.

2.0 Standard Operating Procedures MHU:

2.1 Mobile Health Unit Working Areas:


In order to cover all the flood-affected areas of South Punjab, Sindh, Baluchistan, and KPK, MHUs
activity will be driven in cluster form and each cluster will cover its adjacent districts. Following
are the clusters and their areas;

All of the mobile units will be parked in predefined parking areas at these clusters and also refill
inventory from these base points.

2.2 Arrangements:
a) Mobile Health Unit will rotate on a daily basis in flood-affected areas (in cluster form).
b) The schedule of mobility of MHU will be provided by the regional team on monthly basis.
c) Tents and chairs for the waiting area of patients outside the MHU will be arranged by the
local team of Alkhidmat.
d) Clean drinking water for MHU staff and patients will also be arranged by the local team
of Alkhidmat.

2.3 Advertisement of MHU:


MHU In-charge and Regional Coordinator is responsible for the advertisement of MHU through
following means:
a) Announcement through Mosques
b) Posters will be displayed at Hospitals
c) Banners will be hung in crowdy areas

d) Social Media will be used for announcements in the local area.


e) Camp awareness posts will be circulated through WhatsApp in local groups.

2.4 Working Hours of MHU:

a) The Mobile Health Unit should arrive at the designated place at 9:00 AM and make
necessary arrangements for the operations.
b) The working Hours of MHU will 6 hours (9:00AM – 3:00PM). Extendable in case of
emergency.

2.5 Storage of Refrigerated Items

a) Store all the Temperature Refrigerated items at 2°C to 8°C.


b) Temperature must be maintained all the time in the refrigerator of MHU and in the
refrigerator of warehouse.
c) Make sure the Temperature of Refrigerated items.

2.6 Data Reporting:

Every mobile health unit incharge will make sure the data entry of patients in MHU software,
generate the daily report and share with manager mobile health unit.

3.2 Monthly Salary


a) A person who is hired in the Mobile Health Unit Project would be paid by the head office
(AHF). An individual’s salary is to be disbursed according to the terms and conditions
mentioned in his contract letter through bank account.

All employees, except daily wagers would be paid through cross cheques issued in their name or
directly bank transfer. At the time of approval of employees’ salary expense for the month, a
detailed report for each employee will be prepared containing the following information:

b) The aforesaid report will be furnished to Managing Director (AHF) by the Senior Manager
Finance for necessary approval, before approving salary disbursements Senior Manager
Finance would arrange comparison of gross salary with last month and obtain satisfactory
reasons for change, if any. After the approval, the finance officer will forward the cheques
of Salary Bill to Signatories for his signatures, along with the Salary Letter addressed to
respective Bank, mentioning all the staff accounts with their net salary for credit in their
respective bank accounts.

3.2.1 Payroll Taxes


a) At the time of salary disbursement, income tax will be deducted as per Income Tax
Ordinance, 2001. Thereafter, due income tax will be deposited into the Government
treasury through tax challan.

b) The deducted amount of income tax would be deposited as per stipulated time period
mentioned in Income Tax Ordinance.
c) Periodic tax statements under the Income Tax Ordinance will be prepared within the
stipulated time.

3.3 Issuance of Advances and Adjustment Method


a) For every payment on prescribed request form shall be forwarded to Senior Manager
Finance after the approval of Managing Director through DM Supply Chain. Request made
verbally or addressed to any other person of Finance Department will not be entertained.
(Annexure Attached)

b) An advance of amounting Rs 100,000/- would be issued to Mobile Health Unit In charge


through bank transfer for daily expenses. This advance would be adjusted on regular
basis. Fresh advance would be issued after submission of previous bills of mounting
Rs. 70,000/-. (Annexure Attached)

c) Monthly limits for expenses against Staff Entertainment would be amounting


Rs 50,000/-. Original bills would be submitted to finance department through DM
Supply Chain.

d) Fueling of Mobile Health units would be made through Fuel Cards. Fuel cards would be
issued after registration of vehicle by the head office, fuel expenses would be adjusted
through advances till issuance of fuel card. Original bills would be submitted to finance
department through DM Supply Chain.

e) Expenses regarding repair and maintenance of mobile health unit would be adjusted
through advance issued to Mobile Health Unit In charge. Original bills would be submitted
to finance department through DM Supply Chain.

f) In case of shortage of any item mobile health unit Incharge can purchase directly with the
prior approval of deputy manager supply chain endorsed by manager mobile health units.

4.4 Waste Management


4.4.1 Clinical Waste

a) All Clinical Waste (Contaminated/Biohazard/Infectious Waste) must be securely packed in


Plastic Disposable Bags store in the designated area and then handed over to the waste
management Company.
b) Housekeepers will use PPE properly and focus on hand hygiene regularly.
c) To prevent aerosol Clinical Waste don’t be dumped as routine garbage or in any way
transferred from one bag into another bag. It must be transported or handed over to
waste management Company in original bags/containers.
d) Needles must be discarded safely and immediately into a Danger box / ‘sharps’ container.

e) Danger boxes should be filled three quarters and packed with red bag by technical staff
and clearly labeled ‘sharps’ before handed to housekeeping staff.
f) All infectious waste containers must be the close lid and lined with bags to avoid any
leakage during transportation. Paper and other packing material handled with gloves /
soiled with blood or body fluid should be discarded as infectious waste.

h) The Waste bags must be grasped and lifted by the knot in the top of the bag and held
away from the body to avoid potential contamination of the handler while transferring to
the transport cart.
i) Waste bags never be thrown or kicked.
j) General waste is collected at different frequencies in the diagnostic center. However,
infectious waste collection is done sometimes more than required frequency
k) All records of infectious waste are documenting on the form.
l) Waste Management Procedures of our lab are implemented and monitored.

4.4.2 Non-Clinical Waste

a) All Non-Clinical Waste (Non-Contaminated, Non-Infectious Waste) should be packed in


Plastic Disposable Bags; this type of waste includes stationery items, all type of papers, food
items, used tissue, used alcohol swabs/wipes, all type of packaging materials etc.

4.4.3 Contaminated / Hazardous Waste:

a) It is the waste arising from resulting from the investigation, testing or treatment of
patients.
b) It is the responsibility of technical staff to identify the waste as contaminated or
hazardous, labeling and properly treating it before disposal.

4.4.4 Step to be followed for Clinical Liquid Waste Disposal:

a) All liquid waste from the instrument is stored in a large size plastic container.
b) Add sodium hypochlorite 5 % into the liquid waste container at a ratio of 25 % (125ml
per 500ml liquid waste.)
c) Let it stand for 30 minutes
d) Dispose of in normal drain with plenty of water.

4.4.5 Precautions for safe clinical liquid waste disposal:

a) The collection and solidification of clinical liquid waste should be conducted at point-of-
generation.
b) Be familiar and have education about the principles of handling infectious waste and
infection control.
c) Practice frequent and appropriate hand washing.
d) Stress that staff is adequately trained for disposing of clinical liquid waste disposal and
handling.

4.5 Monitoring & Evaluation


a) Ensure patients per day are properly registered.
b) Electricity along with 6.5 KVA generators for backup available.
c) Generator Oil, air filter, and batteries health reports
d) Safe with a shade location for parking of the vehicle.
e) Adequate security arrangements for MHU.
f) Administrative arrangements for staff should be properly arranged.
g) Weekly Report about the millage of trucks with locations through the tracking system.
h) Equipment performance reports (Lab)
i) In case of any incident detailed report should be shared with the manager MHU on
same day.
j) Head office team will plan and do surprise visits and to make sure the compliances.

5.0 WAREHOUSE FOR MHUs

Due to Devastated Floods in Pakistan, there is a huge loss of lives and infrastructure. Millions of
people need help in form of food, healthcare facilities, and shelter. There are a lot of healthcare
facilities that are partially or fully damaged, leaving people helpless in medical conditions that
are prevalent in flood affected areas due to multiple reasons.

One of the many challenges faced, is to establish uninterrupted supply lines for all the unit. Major
sectors of this supply line are medicines and laboratory kits & consumables. A comprehensive
plan is required to address this issue especially in areas where MHUs are operating. As it is not
possible to store entire month’s stock on MHU. There is a need for opening a sub-store in the

operational area for stock storage and MHU filling. Sub-store will be managed by the MHU in
charge. Due to workload, the MHU in-charge will not be able to handle large quantities of stock
and if a small quantity of stock will be stored then the shortages will arise due to procurement
and transportation issues.

One solution to this problem, is to establish region-wise central warehouse. Central warehouse
will be responsible for receiving and distribution of stocks to sub-stores where MHUs will be
functional. Central Warehouse will also be linked with sub-stores for having check on
consumption patterns and emerging requirements.

5.1 Requirements for Establishing Warehouse:

Legal and material requirements will be fulfilled according to regional legislations. Following are
the major requirements of the project

I. Area: A covered space of appropriate dimensions (600sqft) and location should be


acquired. The space should have appropriate access to the main roads for stock
transportation. Moreover, the location should be easily accessible to deputed staff. Basic
facilities like electricity, sewerage, and water supply lines should be present.

II. Fix assets: following fixed assets will be required for operations
a) Air conditioner for maintaining the storage area temperature in prescribed limits.
b) Racks for proper storage and display of medicines and consumables.
c) Fridge for storage of temperature-sensitive items.
d) Temperature meter for measuring the room and fridge temperature.
e) Working Desks for office work and for checking and packing of stock.
f) Ups for uninterrupted Power supply.

5.2 Working of Warehouse:

Warehouse Incharge will be appointed in the regional warehouse will look after stocks. Stock will
be received and a sheet will be maintained showing the issuance status and in hand stock status.
regional warehouse will issue the stock to sub-stores (where MHUs will be operational) for
predefined days. MHU in-charge at sub-stores will receive the stock, maintain inventory and issue

Job Purpose:

Manager Mobile Health Units will be responsible for managing his office at the central level,
preparing guidelines for mobile health units, coordinating and monitoring regions, consolidating

I. The Manager Mobile Health Units Program is responsible for the overall operational
management of the mobile Health Units.

I. Provide patient healthcare at mobile clinics implemented in flood-affected areas

IV. Prescribes medicines to be taken at home.
V. Explains to the patients about the use and effects of prescribed drugs.
