ANNOTATION

A. INSTRUCTIONAL LEADERSHIP

1. Developed SLCP

ANNOTATION: Aligned with DepEd’s Sulong Edukalidad. MNHS-SHS provided a


learning continuity plan wherein its primary objective seeks to ensure student’s learning
progress even amidst of Covid-19 pandemic. One of the highlights of this SLCP is to
overcome obstacles created by the said disaster through innovative means of teaching
and learning, keeping students on track with their courses.

2. Modular Learning Modality

ANNOTATION: MNHS-SHS Academic Year 2020-2021 provides another alternative


learning modality for the new normal which is also stipulated in the SLCP which is
Modular Distance Learning wherein it allows learners to use self-learning modules
(SLM’s) in print or digital format/electronic copy, whichever is applicable to the learner.

3. AIP/SIP
ANNOTATION: MNHS-SHS – push all participating channels to combine the nuggets of
wisdom and efforts for the success of SHS Enhanced Implementation Plan (ESIP). The
SHS and all its tracks and strands together with the teaching force by the SIP Planning
Team center its goal to meet quality and complete education to all learners.

4. APP
ANNOTATION: Our Annual Procurement Plan (APP) provide information on significant
procurements entities plan to undertake over the next 12 months. APP’s of SHS include
a concise strategic procurement outlook statement, which broadly discusses key, major
or strategic initiatives from which the entity expects procurements to arise.

5. INSTRUCTIONAL SUPERVISORY PLAN


ANNOTATION: The table shows, the Instructional Supervisory Plan of SHS wherein all
strand already complied their scheduled observation along with the different subject
group heads. It only show that indeed this activities or actions with the respective
supervisor takes to promote growth in teachers as well as students learning.

1. WORK PLAN OF THE PROGRAMS & PROJECTS


ANNOTATION: MNHS-SHS together with competent teachers and faculty members
developed different projects such as Project I-CURE and Project E-CLIK. Presented are
the work plan and programs of the said project wherein its main competency are as
follows: for Project I-CURE School should be able to apply research findings to atleast
75% of the intended beneficiary by 2022. And for project E-CLIK the school should attain
at least 75% average of Learners Oriented Assessment in Mathematics subject as a
result of the initiated intervention.

3. ACCOMPLISHMENT REPORT/LOA RESULT


ANNOTATION: SHS prepared a status report that provides an overview of what was
achieved during the School Year 2020-2021. It shows the range of grades (number and
percentage), its mean percentage etc. Therefore, it connotes to determine the extent to
which graduates of the program are achieving the major educational outcomes intended
by the faculty.

1. M&E Tools, SLCP Implementation Plan
ANNOTATION: MNHS-SHS provided a checklist on the presentation of school learning
continuity plan wherein it facilitates the execution of a strategic plan/project by breaking
down the implementation process through checklist. While defining its timeline, the
teams and the resources will be also needed.

2. Situational Analysis School Memo and Narrative Report on Performance Review


ANNOTATION: MNHS-SHS conduct its “Kamustuhan/Midyear Performance Review on
the different projects implemented by the school. Wherein its main objective is to review
the status of the School Project Implementation for school Year 2020-2021. Moreover,
the said virtual meeting achieved its objectives.

SCHOOL LEADERSHIP, MANAGEMENT AND OPERATIONS (OBJECTIVE 1)


1. LIQUIDATION REPORTS
ANNOTATION: Duly signed by yours truly together with our Administrative Officer II,
presented here is the Liquidation Report for school year 2020-2021 which highlights the
following: Funding Source from MNHS-SHS and funding source from division office.
Therefore, its main function is to make sure that a company is wound-up equitably and
fairly.

2. UPDATED TRANSPARENCY BOARD


ANNOTATION: Transparency is given prime prioritize by MNHS-SHS for we wanted to
ensure that public funds are properly utilized for what it is intended for, that is, public
service. So that faculty members and its stakeholders are aware and vigilant continually
monitoring how public funds is spent.

SCHOOL LEADERSHIP, MANAGEMENT AND OPERATIONS (OBJECTIVE 3)
1. TEACHERS’ CLASS SCHEDULE
ANNOTATION: The institution provided a teacher’s class schedule wherein routines
also help the teacher build a relationship with the student, helps them when planning
and also helps with child guidance.

2. CLASS PROGRAM
ANNOTATION: Attached herewith is the class program and schedule that sets the
stage for teachers to communicate to students exactly what is expected of them and
allows the teacher to identify behaviors that must be directly taught to one or more
students, which, in turn, helps them to develop greater student autonomy, responsibility,
and self-control.

3. ACCOMPLISHMENT REPORTS
ANNOTATION: Presented, is the accomplishment reports on the following enhanced
school improvement plan (ESIP) for school year 2020-2021. Wherein it aims at putting
the best interest of the child especially the poor learner, at heart by understanding the
child himself and his interaction with his environment (the home, the school and the
community) through a standardized teaching system.)

4. TEACHING LOADS
ANNOTATION: This artifact shows the teaching loads of teacher wherein they are given
a 6hrs load and 2 hours of paper works. It only shows that MNHS-SHS follow the
standard given by DepEd as stipulated in its memorandum.

LEARNING ENVIRONMENT (OBJECTIVE 1)


1. TA/COACHING PLAN
ANNOTATION: This artifact is a technical assistance form given to one of the faculty
members of the institution wherein one intervention is the assistance via messenger on
the templates and forms needed to be submitted for BERF application about research. In
this regard, sharing of information and expertise, instruction, skills, training, transmission
of working knowledge, and consulting services also involve the transfer of technical data.

2. SCHOOL MEMORANDUM ON SLAC/INSET


ANNOTATION: Presented here is the school memorandum on Virtual In-Service
Training (INSET) with the theme: “Remote but Connected: Gearing Up for the New
Normal.” Wherein, topics covered are the following: Best Practices for Distance Learning
Classes, Common Technical Problems in Distance Learning Classes with Solutions,
Research ICT Tools, BERF, Professional Advancement Options, and Stress
Management Techniques for Teachers, Remediation Protocols and LAC for New
Subject Assignments.

3. SLAC/INSET DOCUMENTATION
ANNOTATION: This photo signifies that SLAC/Inset was a successful one, wherein
teachers are very active sharing their experiences during their time teaching in the new
normal situation. Despite of the circumstances still teachers of SHS can still manage
their time in participating this one of a kind training.

ANNOTATION: The artifact given is a coaching report of some faculty members of
MNHS-SHS with regard to the different projects implemented by the teachers written in
ESIP. Proper intervention was addressed and it integrates the information from the
potential, challenge, and core values reports into a 5-step planning process.

LEARNING ENVIRONMENT (OBJECTIVE 2)


1. WHLP
ANNOTATION: Attached is the sample artifact of WHLP of MNHS-SHS Grade 11
Learners. Wherein, the Day &Time, Learning Area its competency, the learning Task
and the Mode of Delivery were included on the said plan to ensure that students will
properly follow and would serve as a guide all throughout in answering their modules.

2. WALKTHROUGH MODULES
ANNOTATION: This artifact is a school memorandum about self-learning module
walkthrough happened last September 07-11, 2020, wherein its primary objectives are
the following: to check if there are activities in the modules that need to be
contextualized. To check the alignment of SDO-Rizal’s crafted test items with the
competencies discussed in the modules and if they are congruent regarding on the
suggested performance outputs to the activities in the modules. Also, pictures of writers
are included in this artifact wherein they received a certificate of recognition and
appreciation.
