1st Agenda:

Brief run-through of ISO, monitoring of task assigned before the said Audit is a must. Don’t be
complacent and be responsible enough.

2nd Agenda:

SICU and Burn ENCAR

It was then observed by M’ Dina that the special areas of ward 4 does not follow the prescribed gown for
watchers and patients. A re-orientation of basic policies and requirements. Guidelines should be
followed on transfers to and from another department. Ma’am Rose to check and put stickers on gowns
for labeling process. Morning shifts should check and change gowns before or during the start of their
shift.

3rd Agenda:

Leave application concerns on filing and submission. Time Adjustments from time to time, check if you
have errors on HRBliz Prior to the date submission of DTRs.

Ward 4 checklist are blank. Documents and data entry are lacking. Lab results lacking. Always check
doctors’ orders. Do endorsements and rounds.

Please communicate with your colleagues and understand the intention of their message when being
questioned with work related events.

A quick voting was arranged on whether to penalized deficiencies on charts. It was then decided that
20 pesos per chart with deficiency will be collected. 10 consecutive deficiencies will be subjected to
negligence and may result to ENCAR. Failure to comply within 24 hours will be considered as
deficiency It was also settled that these deficiencies and negligence will be forwarded to the Chief
Nurse for disciplinary action. The amount collected will then be allocated to our Hospital Funds.

Medication should follow standard medication rules, unless it is otherwise with special orders from
doctors. Double bar:shifted/discharge/re-copied:date:initial is the standard way of charting
medications.

Re-iteration on patients that are Expired without sanitation charges.

A fine of 100 pesos for those who failed to attend the said meeting, on duty are excused as well as those
who are on leave and on their day-offs.
4th Agenda:

Nursing Attendants

There is a proper arrangement pertaining on the photos on the directories of schedule (whiteboard).

Nursing Attendants should not solely administer the dressing of burn patients––they can only assist
the Nurse on duty.

Mr. Rebollos suggested that there should be a regular rotation of Nursing Attendants––the body
agreed.

Rotations on NA’s burn dressing will start this incoming month of September.

Communicate well and express well.

Route and frequency reminder, not usually on night shift nurses/na’s

It is still the NOD’s responsibility to encode meds.

NOD’s must coordinate with NA’s with other related task.

Absari ask if suction on SICU included in their responsibilities. Aiko answered, NO!

Green Space to initiate ASAP
