Procedure:
1. Shade the appropriate circle for each question.
2. Add the score (4 strongly agree, 3 agree, 2 disagree, 1 strongly disagree) for each response to get the total for
each wellness dimension.
3. Add all the total scores from all wellness dimensions to get the score for your overall wellbeing.
4. Score each wellness category using the ratings table.
5. Complete the conclusions and implications sections.
