Task 3
The task is to gather case histories
from five different individuals and
also perform Mental Status Exam
(MSE) on them. The case history
and MSE format is provided for
the task. Please utilize this format
to collect case histories from five
different individuals and ask them
to take the MSE exam.
 Task Details

Use the case history format to conduct an

interview or administer a questionnaire to
gather relevant information from each
individual.

Practice active listening during the interview

to ensure accurate and detailed information is
collected. Encourage the individual to share
openly and ask follow-up questions for
clarification if needed.

After the interview, ask them to give the MSE

test and collect the respective response sheet.
General Instructions
1. Accurately document the responses from each individual in the MSE and case history
format.
2. Ensure the collected information remains confidential and is not shared with your friends
or colleagues.
3. Organize the report by presenting each individual's case history separately.
4. Identify the individual's strengths, coping mechanisms, and areas of resilience.
5. Acknowledge any positive aspects or resources that can contribute to their well-being.
6. Conclude the report by summarizing the key findings and implications.
7. Upload the report word file while submitting the task.
