Parent’s Middle Name is not indicated – ask for BC of parent, if no BC, let it be just an initial

Erasures – neat and with initials
Grades of dropped – must be entered up to the last grading period completed
*ALL checked forms must be attached with CERTIFICATION to be issued by the Guidance Office.
