Dear PD Dacuma:

I am John Edward A. Raagas, Instructor at Monark Foundation Inc. and an accredited

Competency Assessor of Heavy Equipment Operation (HEO) Forklift NC II. This letter pertains
to the incident transcribed last October 7, 2022, wherein I received a notification from Ms.
Sheryl R. Mendiola, CAC Manager -PIETI , that I was scheduled to conduct the Forklift NC 2
assessment in their center (October 7, 2022); however, I was also scheduled that day to
conduct an NC 2 assessment on the same qualification in Monark Foundation, Inc.

Last September 26, 2022 Ms. Sheryl R. Mendiola invited me via messenger to conduct a Forklift
NC 2 assessment in PIETI for October 7, 2022. I declined the invitation, as I was already
committed to conduct the assessment at Monark Foundation Inc (MFI) on the said date. Based
on T2MIS, I was scheduled to conduct Forklift NC 2 assessment in Monark Foundation Inc. on
October 7, 2022 and not in PIETI. I had no idea that PIETI proceeded with the request although
I did not confirm my attendance for the said assessment.

I was advised by Mr. Ian Moses Sanchez to submit an explanation letter to clarify the matter. My
apology if this caused any inconvenience in TESDA and other parties involved. Rest assured
that proper coordination will be observed next time to avoid such inconveniences.
