Kitts and Nevis Travel Form

We strongly encourage RUSVM students to follow the instructions closely to ensure your application for
entry to the island is prioritized and not delayed unnecessarily.

• Due before travel: St. Kitts and Nevis Travel Form https://www.knatravelform.kn/
• Due no later than 72 hours before travel: Upload your negative COVID-19 test result into your

• Due before you board your flight to St. Kitts: Screenshot or print your government approval
letter

All portions of the survey should be completed before you depart for St. Kitts. In order to board your
flight, you will need to have an approval letter from the St. Kitts Government. If you test positive for
COVID-19, please notify David Satterlee, Director of Student Experience, at dsatterlee@rossvet.edu.kn.

All required fields are indicated by an asterisk symbol (*). Please note that the information you provide
will be kept confidential.

2) Contact Information

Enter your valid email address and phone number

Please use your RUSVM-issued email address.

3) Destination Information

Purpose of Visit: select “Study”

Length of Stay: enter “90+”

Type of Accommodation (after 14 days): If you are renting an apartment/house, select private home.
Additional fields will then be required. Enter the name of your landlord and your landlord's contact
information. If you will be staying in Campus Residences, choose RUSVM Campus.

Intended Address in St. Kitts and Nevis: the address where you will be living after quarantine period

Island: choose “St. Kitts”

Village/Town: select “Basseterre”

Host Name: enter your landlord’s name

Host Contact Number: enter your landlord’s phone number

Site for Vacation in Place / Quarantine Site: select “no quarantine” if you are a fully vaccinated traveler
entering St. Kitts; if you are not vaccinated, select your pre-approved Quarantine Site

3) Passport Information

IMPORTANT NOTE: Your passport information is the only valid form of identification. Please select
“Passport” as your Travel Document Type. Do not enter your driver’s license.

You will be asked to include information from your bio-data page. This is the page in your passport that
includes biographical data (name, date of birth, passport number, expiration date, etc.). Most passports
will also show your picture on this page.

2. Upload your acceptance letter to RUSVM (new students) or upload copy
of Ross ID for (returning students)

Note: In order to minimize any delays in processing, submit one (1) PDF/JPEG that includes your bio-
data page and your acceptance letter or Ross ID.

4) PCR-Test

All travelers must submit a COVID-19 RT-PCR negative test result from a CLIA/CDC/UKAS approved lab
accredited with ISO/IEO 17025 standard, taken 72 hours prior to travel, along with the required
embarkation form, and all other supporting documentation. Forms must be submitted no later than 24
hours prior to travel.

5) Proof of Vaccination

To be eligible for the St. Kitts government’s “Land and Go” protocol allowing re-entry without needing to
quarantine, you must upload a photo of your vaccination record/card. There will be a section on the
form to do so. If that section is unavailable, in your PDF upload include both your bio-data page AND
your vaccination record/card along with acceptance letter or Ross ID.
