1. Purpose
The purpose of this document is to establish and maintain a structured procedure for the
ongoing hazard / Aspect identification, risk assessment / Impact, and determination of
the necessary controls that meets the OHSAS 1800:2007 and ISO 14001:2004
requirements.
2. Scope
This procedure will be implemented in all Sarooj Construction activities.
3. Key Requirements
 This procedure for Hazard / Aspect identification and Risk / Impact assessment shall
take the following into account:
 Routine and non-routine activities.
 Activities of all persons having access to the work place (including contractors and
visitors).
 Human behaviours, capabilities and other human factors.
 Identified Hazards / Aspects originating outside the work place capable of adversely
affecting the health and safety of persons under the control of the organisation
within the work place.
 Hazards created in the vicinity of the workplace by work related activities under the
control of the SCC organisation. Note: Such hazards to be assessed as an
environmental aspect.
 Infrastructure, equipment and materials at the workplace, whether provided by the
SCC or others.
 Changes or proposed changes in the SCC organisation, its activities, or material.
 Modification to the OH&S & Environmental management system, including
temporary changes, and their impacts on operations, processes and activities.
 Any applicable legal obligations relating to risk assessment and implementation of
necessary controls.

 The design of work areas, processes, installations, machinery/equipment, operating


procedures and work organization, including their adaption to human capabilities.
4. Methodology

SCC’s methodology for Hazard / Aspect identification and risk / Impact assessment shall:
 Be defined with respect to its scope, nature and timing to ensure it is pro active
rather than reactive; and
 Provide for identification, prioritization and documentation of risks, and the
application of controls, as appropriate.
For management of change, the SCC shall identify the OH&S hazards / Environmental
Aspects and OH&S risks / Environmental Impacts associated with the changes in the
SCC, the HSE Management system, or its activities, prior to the introduction of these
changes. SCC shall ensure that the results of these assessments are considered when
determining controls.

When determining controls, or considering changes to existing controls, consideration


shall be given to reducing risk / impact according to following hierarchy:

1. Elimination

2. Substitution.

3. Engineering controls.

4. Administrative controls, signage/ warnings.


5. Personal Protective Equipment.

SCC shall document and keep results of identification of hazards / Aspects, risk /
Impacts assessment and determining controls up to date. SCC shall ensure that OH&S
risks / Impacts and determining controls are taken into account when establishing,
implementing and maintaining its HSE Management System.

1. PROJECT MANAGERS ARE RESPONSIBLE FOR:
 Ensuring that, in their area of responsibility, Hazard / Aspect identification, Risks /
Impact assessment and determining controls for all activities in their respective
Projects are in place. These should be documented, maintained and reviewed
regularly.
 Monitoring the consistency of implementation of the Risk / Impact assessment
process.
2. CORPORATE HSE DEPT IS RESPONSIBLE FOR:
 Supporting the Project Manager in developing the Hazard/Aspect identification, Risk /
Impact assessment and determining controls.

8. Process
SCC has adopted the HSE Management model to Risk Management. Hazard / Aspect
Identification, Risk / Impact assessment form “A”, Consequence Table 1, Likelihood
Table 2, and Overall Risk / Impact rating Table 3 shall be used to identify all
Hazards/Aspects and associated Risks/Impact and controls Risks/Impact.

The process has four basic stages:


I. Identifying the OH&S hazard/environmental aspects and associate OH&S
risks/environmental impacts
II. Conduct risk rating as per rating matrix below.
III. Identify controls and conduct risk rating again.
IV. Assessing the OH&S and environmental residual risks/impact
V. Monitoring and Review

b) Identifying the Likelihood


The next step is to determine the likelihood that such an outcome will occur. This
involves two steps – determining the frequency of exposure and the probability of an
incident occurring and looking up the resulting likelihood in table 2 below. The basic
premise is that the more often an individual is exposed to a hazard, the more likely it is
that an incident will occur.

C) Assigning a Risk Rating


Having decided the consequence and the likelihood, the risk rating is determined by
using table 3 below. The risk rating is determined b reading the likelihood on the left-
hand column of the table against the consequence across the top row of the table.

Using this process, all of the hazards associated with the task, job, workgroup or
workplace can be assessed and a risk rating assigned to each.

c) Table 4 below indicates the acceptability of the assessed risk level and any additional
workplace requirements or implications.

Note: Where you observe an immediate risk to health or safety of anyone in the
workplace; remove yourself and others (cease the work if appropriate), and notify the
work supervisor immediately. The supervisor should ensure the current work method is
revised to ensure that adequate risk controls are put in place.

9. Residual Risk
Monitoring of the controls is important to ensure that the control measures put in place
meet acceptable standards and do not create hazards themselves. This can be
accomplished by audits, re-assessments, analysis of incident reports and through
proactive considerations by management and employees.
A review of risk assessments and any measures adopted to control the risk will be
undertaken whenever there is:
 Evidence that the risk assessment is no longer valid,
 An injury or illness resulting from exposure to a hazard to which the risk
assessment relates,
 When new information regarding the hazard becomes available,
 A significant change proposed in the place of work, in work practices or
procedures, or
 A new plant, equipment or hazardous substance is introduced into a process or
workplace.
Risk assessment information shall be reviewed as required and as a minimum every
year.
10. Communication
Relevant information regarding the hazards and risks associated with the workplace shall
be communicated to employees and contractors in a timely manner. This includes the
induction of new employees.
11. Records
Risk assessment records must be retained for a period not less than five (5) years.

Each Project will maintain documentation relating to their risk assessments of the
identified hazards, risk assessments, control measures and monitoring information,
associated with that area.

4. HIERACHY OF RISK / IMPACT CONTROLS:


The order for implementing risk control measures with the most desirable being hazard
elimination, the least desirable being the provision of PPE
a) Elimination – remove the hazard / aspect completely during design or later;
b) Substitution – replace the hazard / aspect completely during design or later;
c) Isolation / Engineering controls – measures to reduce exposure e.g. install guards on
machinery, fume extraction, remote controls. Etc.
d) Administration – signs and procedures for safe practices, training, supervision, etc.
e) Personal protective equipment (PPE) – respirators, ear plugs, gloves, etc.
