Major Claim Processing Procedure

1. Policy holder / dependant / Heir can submit the claim by filling out AIA Claim Form and enclose all
supporting documents, in accordance to claim type that can be seen on the Major Claim
Document Requirements menu, no later than ninety (90) calendar days since the date of
decease or accident or the date where the diagnosis on the critical illness or disability is given.
2. The claim documents can be submitted to AIAF Central Office at the address, Menara Palma 21st
Floor, Jl. H.R. Rasuna Said Blok X2, Kav. 6, South Jakarta, Postal Code 12950, Indonesia
through HRD of Policy Holder
3. Documents that fail to comply with the requirements will result to claim pending and the
documents shall be returned through HRD of Policy Holder, to be completed by the dependant /
heir
4. The completed documents can then be re-submitted to AIAF Central Office at maximum 30 days
since date of document return notification
5. AIAF will process the claim documents that are considered to be complete in 14 working days
time, since the date where the documents have been completely received by AIAF Central Office
