A photocopy of your PhilGEPs Registration Certificate/ Mayor’s Permit / DTI / BIR Registration/Income
or Business Tax Return/ Omnibus Sworn Statement in accordance with Section 25.3 of the 2016 Revised
IRR of R.A. No. 9184 are also required to be submitted along with your quotation /proposal.

TERMS AND
CONDITIONS

1. All entries must be typewritten or legibly written.

2. The sealed Request for Quotation must be submitted or ready for pick up on or before the date &
time stated above.
3. Price Quotation should be valid until December 31, 2023.
4. Price Quotation should be inclusive of tax.
5. Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by
you or any of your duly authorized representative/s.
6. Quotations exceeding the Approved Budget for the Contract shall be rejected.
7. The approved budget ceiling for this procurement is Php393,000.00.
8. Evaluation of the quoted price will be conducted on the basis of the total price.
