Proposal and Thesis Supervision Form

This proposal and thesis supervision form is for use by students of the English Literature Study Program, Faculty of Cultural Sciences, Universitas
Sumatera Utara, who have obtained their thesis supervision decree letter (SK pembimbing skripsi).

The following rules are adopted from the Academic Regulations of USU No. 13/2022 and the Faculty of Cultural Sciences Guidebook for Thesis
Writing (2023). Please observe them to ensure the smooth completion of your proposal/thesis.

REGULATIONS:
1. You must conduct thesis supervision at least eight times with EACH supervisor, starting from the proposal seminar preparation to the final
defense. Both supervisors MUST approve the draft of the proposal/thesis before you can submit it for proposal seminar or thesis defense.
2. No draft revisions are accepted after the proposal/thesis exam is scheduled. The Program reserves the right to postpone a scheduled exam if
students violate this rule.
3. The thesis topic will be canceled if you showed no progress nor had any supervision with your supervisors within a maximum of 6 months after
the supervision decree letter was issued. You must then submit a new topic and restart the supervision process from the beginning.

INSTRUCTIONS (1-6):
1. Fill out your full name, ID (NIM), academic advisor’s name, and advisor’s staff ID (NIP).
2. In column (1), fill out the date of the consultation.
3. In column (2), write feedback/comments from your supervisor.
4. In column (3), the supervisor signs as proof of supervision.
5. You are responsible for the safekeeping of your supervision form, and providing a copy for your supervisor and the Program.
6. You may print/copy the supervision page as needed.
