I.

SCHEDULE OF M&E

To assess and monitor the progress of each projects, programs and activities
of the school, the Monitoring and Evaluation team conducted face to face pre-
monitoring activities prior to the undertaking monitoring activities including the
site-visits where different projects implemented. Meetings convened by the
team in connection with the projects, programs, and activities. Cluster meetings
within the people behind each project were also observed. On conducting the
pre-monitoring activities, the different project proposals were reviewed.
Adequate preparation prior to the monitoring visit provides grant the team with
the background information relative to the stated goals and objectives necessary
to conduct a thorough visit. The Monitoring and Evaluation team scheduled the
monitoring on-site visits. Each leader for each project encourages to review the
technical assistance given by the team. On the on- site monitoring, the team
has the checklist to determine if the project is ongoing on its necessary timeline
and achieving its interims goals and objectives. The post-monitoring will be
schedule to be done at the end of the school year since most of the projects,
programs, and activities are still on going.

Thus, monitoring and evaluation work involves data gathering and data
collection. Data will be collected through monitoring and evaluation tools
disseminated from national, regional, division and district offices. The school
will also utilize modified monitoring and evaluation tools in its PAPs such as
interviews and observation to ensure that all of the projects, activities and
programs are on track.

At the end of all activities reports for each specific project will be
prepared by the member assigned to do the task and will be consolidated
into a single monitoring and evaluation report. The consolidated reports are
to be kept in the SBM corner and will be displayed in conspicuous area of
the school. The school report card of the school will be disseminated to the
stakeholders as they will also be requested to attend the state of the school
address for everyone’s awareness on the status of the school’s PAPs.

Quality and Equity


A. Pre-Monitoring Activities:

1. English and Filipino teachers tabulate matrix and group reading level for each
learner in the program using PHIL – IRI module.
2. Teachers provide intervention and supplemental activities for every struggling
learners for them to learn the lessons.
3. Measure the numeracy rate by conducting assessment among Grdae 7-10
students and consolidated least learned competencies as basis in crafting the
intervention work sheets.
4. There are 52 identified learners under the reading program with their
coresponding reading and comprehension ability.
5. There 42 indentified non numerates learners under the numeracy program with
their coresponding fundamentals skills and basic algebra.

B. Monitoring Activities:
1. Provide categorized group of the students according to their level of reading and
comprehension through “Project Reading League” school based reading
intervention program.
2. Intensify stakeholders partnership with “AITE” by giving ten Field Study Student
as the school partners in reading intervention and activities.
3. Conduct “peer tutoring “ strategy to facilitate Math Drills and actvities with the
school based numeracy program “Project ML -Mathematics in Action.
4. Some crafted work sheets, localized and contextualized materials and other
interctive activities were utilized through “Project Cuaderno”, specifically in the
reading program.

C. Post Monitoring Activities:


1. The team will utilize the Target vs. Accomplishment tool to plan adjustment on
the implementation of each project.
2. The school will used Unified Monitoring and Evaluation Tool serves as the
guidance of the team for improvement, adjustment and proper implementation of
the activities.
3. The team will include the PAPs result on the school report card and will be posted
on the SBM corner.
