Analysis of Patient Safety Implementation in Health Services in South


Tangerang City

Background

Patient safety has become a priority for hospitals. Patient safety is a system that
provides services to patients without causing injury. Patient safety includes risk assessment,
identification, risk management for patients, reporting and analysis , efforts to carry out
continuous learning, as well as implementing solutions to avoid injury due to negligence,
taking an action or not taking the action that should be taken because almost every medical
action save potential risk. The number of types of drugs, types of examinations and
procedures, as well as a large number of patients and hospital staff, are potential for medical
(medical errors) ( Ministry of Health , 2017).

Patient Safety is a global and national issue, National Patient Safety Agency 2017
reported that in the period January – December 2016 the number of patient safety incidents
reported from the UK was 1,879,822 incidents. Reports from acquired hospitals in the world
from 2011–2014 found unwanted events including, pressure ulcers 42,716 incidents,
medication errors 16,760 incidents, catheter use infections 7,922 incidents, others 7,746
incidents, bloodstream infections 4,403 incidents, patient falls 2750 incidents, postoperative
venous thrombolysis 1,768 incidents, surgical side infections 1,692 incidents, ventilators
regarding pneumonia 1150 incidents and obstetric side effects 26 incidents (AHRQ, 2016).

In Indonesia, according to a report obtained through by the National Hospital Patient


Safety Committee in 2020 report by incident, there were 1689 near-injury events, 1536 non-
injury incidents, and 1722 patient safety incident reports (National Hospital Patient Safety
Committee, 2020).

In South Tangerang there were Unwanted Events the death of pregnant women due to
bleeding 4 incidents, the death of pregnant women due to other 2 incidents (Banten health
profile 2020). Then there was also an incident of patient safety negligence by officers in the
alleged case of Covid-19 screening engineering (Public Relation General Hospital South
Tangerang City , 2021).

In patient safety, there are many different sources of data that can highlight patient
safety. This includes an incident reporting system; complaint; malpractice claims ; patient
reported outcomes; avoidable death; case record trigger tool; clinical care audits; load loss
study; organizational culture survey; and an audit of sentinel events (WHO, 2020).

Therefore patient safety is priority in health services and there are many sources of
data that can be highlighted for a patient safety incident reporting system, it is necessary to
analyze the application of patient safety in health services in the city of South Tangerang to
ascertain whether the application of patient safety in the city of South Tangerang has been
implemented well.
Analisis Penerapan Patient Safeti pada Pelayanan Kesehatan di Kota Tangerang
Selatan

Latar belakang

Keselamatan pasien (patient safety) saat ini telah menjadi prioritas utama bagi
fasilitas pelayanan kesehatan. Keselamatan pasien merupakan suatu sistem yang memberikan
pelayanan kepada pasien dengan tidak menimbulkan cedera. Keselamatan pasien
meliputi penilaian terjadinya risiko, pengenalan, pengelolaan risiko terhadap pasien,
melaporkan dan menganalisa, usaha melakukan pembelajaran secara berkelanjutan, serta
penerapan solusi agar tidak terjadi cedera akibat kelalaian , melakukan sebuah tindakan
maupun karena tidak melakukan tindakan yang seharusnya dilakukan karena hampir
setiap tindakan medis menyimpan potensi risiko. Banyaknya jenis obat, jenis pemeriksaan
dan prosedur, serta jumlah pasien dan staf RS yang cukup besar, merupakan hal yang
potensial bagi terjadinya kesalahan medis (medical errors) (Kemenkes, 2017).

Keselamatan Pasien (patient safety) merupakan isu global dan nasional, National
Patient Safety Agency 2017 melaporkan dalam rentang waktu Januari – Desember 2016
angka kejadian Keselamatan pasien yang dilaporkan dari negara inggris sebanyak 1.879.822
kejadian. Laporan dari rumah sakit yang terakuisisi di dunia dari tahun 2011 –2014
didapatkan kejadian tidak diinginkan (KTD) meliputi, tekanan pada ulkus 42.716 insiden ,
kejadian salah obat 16.760 insiden , infeksi penggunaan kateter 7.922 insiden, lain-lain 7.746
insiden, infeksi aliran darah 4.403 insiden, kejadian pasien jatuh 2.750 insiden, trombolisma
vena pasca operasi 1.768 insiden, infeksi disisi bedah 1.692 insiden, ventilator mengenai
pneumonia 1.150 insiden dan efek samping kebidanan 26 insiden (AHRQ, 2016).

Di Indonesia, menurut laporan yang diperoleh melalui Komite Keselamatan Pasien


Rumah Sakit (KNKPRS) pada tahun 2020, tercatat Kejadian Nyaris Cidera (KNC) sebesar
1689, kejadian tidak cedera (KTC) sebesar 1536, dan laporan insiden keselamatan pasien
kejadian tidak diinginkan (KTD) sebanyak 1722 (KNKPRS, 2020).

Di Tangerang Selatan terdapat Kejadian Tidak Diinginkan (KTD) kematian ibu hamil
karena pendarahan 4 insiden, kematian ibu hamil karena lain-lain 2 insiden (profil kesehatan
Banten 2020). Kemudian juga terjadi insiden keselamatan pasien kelalaian petugas dalam
dugaan kasus rekayasa screening Covid-19 (Humas RSU Tangerang Selatan, 2021).

Dalam keselamatan pasien, ada banyak sumber data berbeda yang dapat menyoroti
keselamatan pasien. Ini termasuk sistem pelaporan insiden; keluhan; klaim malpraktik; hasil
yang dilaporkan pasien; kematian yang dapat dihindari; alat pemicu catatan kasus; audit
perawatan klinis; studi beban kerugian; survei budaya organisasi; dan audit kejadian sentinel
(WHO, 2020).

Karena keselamatan pasien merupakan prioritas utama dalam pelayanan kesehatan


dan ada banyak sumber data yang dapat disoroti untuk insiden keselamatan pasien maka di
perlukan adanya analisis penerapan keselamatan pasien pada pelayanan kesehatan di kota
Tangerang Selatan untuk memastikan apakah penerapan keselamatan pasien di kota
Tangerang Selatan sudah terlaksana dengan baik.
