Greetings! This has reference to my/our claim for death benefits/separation of my late husband Jesus C.
Cantada Jr. of UP CMO.

As per advice of your good office, we respectfully request and/or move that the said claims/benefits be
individually released/processed to all the next of kin, separately.
It must be noted here that the undersigned, is the lawful spouse, and legitimate children of deceased, Jesus
C. Cantada Jr. and thus automatically and legally shall be the rightful recipient/heir of the benefits of the
latter.

Hence, we are again praying for your compassion to release and approved without further delays, the
benefits due the undersigned and their children.
