To Whom It May Concern:

This is to certify that Ms. Isabella Javier, 14 y/o female patient, with
residence at San Isidro, Isabela, is a patient of Ablebodies Physical Therapy Clinic.
She was endorsed for Physical Therapy by Dr. Abogado of Tuguegarao City
who diagnosed her with Severe Adolescent Idiopathic Scoliosis on May 2020.
She has received a total of 40 sessions already. Patient has improved greatly with
continued Physical Therapy, and is expected to continue treatment to prevent
complications. She is currently undergoing Hydrotherapy as part of her PT
program and is receiving complete PT care from Ablebodies Physical Therapy
Clinic. Pain management program is also incorporated on all her PT sessions.
It is my recommendation for her to visit an Orthopedic Doctor who practices
within Santiago City for further evaluation and management and for possible
fitting of a spinal brace. The wearing of Spinal Brace is subject to the
recommendation of the Orthopedic Doctor who might also consider Spinal
Surgery considering the severity of the patient’s spinal curve.
Continued PT treatment session likewise is recommended to ensure that
the overall health condition will be maintained or improved more, hence, I further
recommend another 24 sessions of Physical Therapy sessions. A re evaluation
will be done every after 6 sessions.
Each Physical Therapy session costs P800.00 and may increase if patient
comes with new complaint.
This certification is issued upon the request of Ms. Javier for whatever
purposes it may serve her.
