2 Is there an approved safe system of work, including engineering calculations, for the lifeline system? 
3 Are rigged lifelines 1/2" (12mm) diameter steel wire rope, secured at each end with 3 cable clamps and sufficiently taut? 
4 Has all personal fall protection equipment been inspected and colour-coded for the applicable quarter? 
5 Are system lifelines in good working order, installed as per the manufacturers instructions? 
6 Are fall protection devices (i.e. anchorages, lifelines, harnesses etc.) in good condition and free from defect? 
7 Are lifelines tagged, and clearly signed to state how many workers are allowed to tie off at any given time? 
8 Where personnel are moving during their work, are 2 lanyards worn, with 1 always clipped on (i.e. 100% tie off)? 
9 Is the correct lanyard being used (restraint to restrict movement, or with shock absorber for falls)? 
10 Are all employees using harnesses wearing them correctly and tied off to a secure point? 
