MONTH-END ACCOMPLISHMENT REPORT

SEPTEMBER 2023

For this month of August, the following activities and programs have been performed by the
School Health Services Office:

 Initiated a consultative meeting with the Board of Trustees and Administration.

 Finalized a written school health plan and submitted to the Principal and School Director.
 Made an inventory, prepared the needed supplies and requested for health kits and
medicines.
 Collected and analyzed student health information and arranged preliminary health
problems list.
 Reviewed/updated emergency plans and procedures.
 Developed/revised system to track referrals for care.
 Updated community health resource files.
 Determined dates for faculty, PTA, school board and other meetings in order to plan
attendance and/or presentations.
 Participated in the General Orientation of all learners and discussed about Routine
Guidelines before, during, and after going to school.
 Prepared Contingency and Communication Plans for COVID response.
 Oriented parent(s)/guardian(s) regarding health service policies and procedures, including
medication administration.
 Secured the Training Certificate of attendance in DOLE-BOSH Seminar in Tuguegarao City.
 Posted infographics and other IEC materials related to Covid-19.
 Distributed signages for posting in bulletin boards in every classroom.
 Conducted a meeting with the maintenance regarding strict implementation of sanitation
and disinfection protocols in all areas of the school.
 Facilitated the proper garbage disposal system around the school campus.
 Set up the isolation room and screening area near the entrance gate.
 Sent a written reminder from the school clinic to parents for the whole school year.
 Contacted the Philippine Red Cross Santiago City Chapter for the reintegration of Basic First
Aid among learners 12-17 years old.
 Published the first monthly health bulletin for the month of August regarding eye health.
  Met with the K-12 advisers regarding the discussion of ways to take care of every learner’s
eye during their homeroom periods.
 Ensured health and safety during the culminating activity of Buwan na Wika on August 31st.
