    Sir:

    Greetings!

    I, the undersigned is currently in Civil Engineering Competency Appraisal courses would like to request
    to you to allow me take special examination of the Achievement test this coming Monday, January 11 to
    Wednesday, January 13 2020. I, the respondent wasn’t able to download the Safe Exam Browser
    application needed for the exam. For some reason the said application cause excessive software damage
    on my computer making me unable to use it as examination tool. Attached herewith are the proof of
    installation failure and software damage on my computer.

    I hope you will take this matter with high importance especially this very hard and trying times of
    pandemic. Thank you sir God bless and stay healthy.
