SSM REPORT PREPARATION

COMPANY NAME

I choose ‘FAQIH Clothing & Accessories’ as a my company name. First name off
course it’s my name as I’m the owner of the company. Clothing means clothes that
fit my company type as my company is more towards selling clothes and
accessories. When customers see the name of my store, of course customers will
know that my store specializes in selling clothing. Other than that, my company also
sells clothing accessories suitable with company name. it’s just simple company
name but we want to let my costumers know my company only sells clothing and
accessories.

COMPANY TYPE

For my company type, I choose selling clothes. For more detail about my company
type is the business code is 47711. The description for this code is retail sale of articles
of clothing, articles of fur and clothing accessories. I decided to sell many types of
clothing but will focus more to jersey. For example original football jersey and maybe
some grade jersey for fans issues and players issues. So there will have many types of
jersey to sell. The main reason why I chose it because I believe there will be high
demand from young people to get their favourite football jersey club. Hence, I will
sell some clothing accessories depending on demand.

STEP by Step Company Registration with Brief Explanation

1.Sign up a new account

a) Go to EzBiz SSM official website : ezbiz.ssm.com.my

b) Press “Sign Up”.

c) Fill all the information marked (*).


After that, press “Register”.
d) Fill in all the information marked (*).
e) Tick / in the “Declaration section”.

f) After completion, press “Save”.

g) The user will receive an email notification for the activation of the Ezbiz user
account.

f) Users need to verify the email in advance by clicking “CLICK HERE TO VERIFY
EMAIL” at the email registered on the Ezbiz Online Portal before signing in on the
Ezbiz portal.
2. EZBIZ USER ACCOUNT ACTIVATION APPLICATION

A. Sign in at the ezbiz.ssm.com.my portal.

a. Enter “USERNAME”

b. Enter “PASSWORD” – you can click the eye icon if you want to see the
password.

c. Enter “VERIFICATION CODE”.

d. Click “Sign In”.

B. Click the user profile and select user verification request.


C. Click add new.

D. Attach the mandatory documents as follows and click “submit”.

1) A copy of the front and back identity card or an unexpired temporary


identification document (if there is no identity card) in the Mykad / MyPR upload
section. (document size does not exceed 3mb, JPEG, PDF)

2) A clear selfie picture with the applicant holding an identity card at neck level in
the Upload Selfie column. (document size does not exceed 3mb, JPEG, PDF)

3) Present one of the official documents such as driver’s license / Passport / Birth
certificate / bill utility or official document in his own name in the Supporting
Document column. (document size does not exceed 3mb, JPEG, PDF)
E. Click submit

F. The user needs to make a user confirmation appointment within 3 days from the
date of the application to activate the user account and please fill in at the
following link.

G. The user needs SSM will contact the user via video call using WhatsApp app on
the date and time set chosen from user before the activation of the user’s account.

3. APLLY E-CONFIRMATION LETTER VIA ECOMM UMP

A) Login to student account at E-COMMUNITY – UNIVERSITI MALAYSIA PAHANG


(ump.edu.my)

a. Click user profile and select user verification request.

b. Select “eConfirmation” menu.

c. Select “NEW APPLICATION”.

d. Follow the selection as below and then click “Apply”.

i. Reason for Application = SSM

ii. Type of Letter = STUDENT CONFIRMATION LETTER (ENGLISH VERSION)

e. Click “View Status” in Main Menu to check the status.

f. After its’s approved, download the student confirmation letter by click “VIEW
LETTER” button.
4. START NEW BUSINESS REGISTRATION

A. Visit ezbiz.ssm.com.my portal

a. Enter “USERNAME”.

b. Enter “PASSWORD”- can click the eye icon if want to see the password

c. Enter the “VERIFICATION CODE” and click “Sign In”.

B. Go to the menu and click “My Business Services”.

a. Click “New Business”


C. “Main Information”

1. Please select “Name Type” either,

a. “Trade Name” – using trade name.

b. “Personal Name” – using your own name as per Identity Card (MyKad/MyPR)

2. Enter “Business Start Date”

3. Enter “Partnership Agreement Date” (if any)

4. Please select “Registration Period” by selecting from 1 year to 5 years.

5. In the “Business Info” section, please select either “Yes” if you agree to
purchase business info, or “No” if you do not agree to purchase business info.

6. The status will indicate “Data Entry”.

D. Please select:

1) “Has Branch(es)?” – click “Yes” if there is a branch or click “No” if there are no

2) “Is Online Seller?” – click “Yes” if the business is online or click “No” if not.
E. “Main Address” and “Mailing Address”

i) Enter all marked rooms (*)

ii) Enter the POSTCODE first and the TOWN list will exist.

iii) If “Mailing Address” is same as “Main Address” press “Copy from Main Address”.

iv) Press the “Next” button.


F. “Business Information”

1) State the type of business in the “Business Description”

2) Click “Add Business Code”.

3) Fill in the type of business in the “Keyword” box and click “Search”

4) Select the relevant “Business Code” at least 1 option and Click “Add selected
Code”
5) Once done, press the “Next” > “Owner Information”

6) Click the icon in “Action” to check or update the owner’s information.

7) The owner’s information will be displayed automatically because the owner has
verified before applying.

8) After updating the swap information press “Update” while if there is no change
of information please press “Close”

9) Press the “Next Button”


G. “Attachment” (if applicable only) – please attach the documents in the required
.pdf format according to the application category as follows:

Care Business Registration Scheme (SPPP)

B40 Group Entrepreneurs

- Proof of approval of receiving National Concerned Assistance (BPN 2.0) for


the B40 or Bantuan Prihatin Rakyat (BPR) group in their own name or No.
Mykad/MyPR applicant.

IPT Students

- A copy of the student’s confirmation letter from the


IPTA/IPTS/Polytechnic/College/Skills Institute/Skills Centre

Click “Choose File” & select the document that needs to be uploaded and click
“Upload Button”.
H. “Fee & Declaration”

1) Check the payment section and mark / in the Declaration Section.

2) Click “Preview” to make a final check.

3) Once you’ve finished checking, please click the “Submit Payment” button for
the payment process and select the applicable payment mode.

4) If there are details to update, please click the “Edit back” button.

I. DOWNLOAD THE E-CERTIFICATE

1) After application is approved, login to EzBiz account and download the business
certificate and business information at ezbiz.ssm.com.my.
