1. Donation Details:

5. Tax Exemption:
The Donor acknowledges that it is the Donor's sole responsibility to comply with any
applicable tax laws or regulations regarding this donation. The Recipient shall provide
the Donor with a donation acknowledgment letter for tax purposes, upon request.
7. Entire Agreement:
This Deed constitutes the entire agreement between the parties with respect to the
subject matter hereof and supersedes any prior agreements, understandings, or
representations, whether written or oral.

signing below. This Deed may be executed in counterparts, each of which shall be
deemed an original.

BEFORE ME, a Notary Public, for and in the City of Pasay, this 25th day of July

This instrument consisting of two (2) pages, including this page on which this
acknowledgment is written refers to a DEED OF DONATION and has been signed by
the parties and their witnesses and sealed with my notarial seal.
This Deed of Acceptance (hereinafter referred to as the "Deed") is made and entered
into by and between [Recipient's Organization Name] (hereinafter referred to as the
"Recipient") and [Donor's Organization Name] (hereinafter referred to as the "Donor")
collectively referred to as the "Parties."
1. Acknowledgment of Donation:
The Recipient hereby acknowledges the receipt and acceptance of the donation made
by the Donor as described in the Deed of Donation dated [date of the Deed of
Donation].
2. Purpose:
The Recipient confirms that the donated item(s) will be used solely for the purposes
consistent with its mission and objectives as set forth in [describe the mission or
objectives of the Recipient organization].
5. Reporting:
The Recipient agrees to provide the Donor with periodic reports, at mutually agreed
intervals, regarding the use and impact of the donated item(s) as well as any significant
developments related to the Recipient's activities.
