On Jan,03, 2015 FFBL employee ,while working with a co-worker at Tank F-061 in Area
4-1, was in process of raising board and bracket scaffold from the third to fourth ring of
plates. This was being conducted within the annular space using the monorail hoist. While
raising a bracket, it snagged the scaffold boards that the employee was standing on and
dislodged the previously set section. This caused the employee to fall approximately 10
meters to the ground. The person was wearing a body harness but failed to hook his
lanyard.
A ZERO Tolerance directives from DGKCC and ICPL that anyone caught not
wearing his full body harness during working on heights will be removed
from site immediately.
Hazard:
Falling from heights can cause severe injury and may lead to fatality
