BASIC INFORMATION OF TRAINEE

I am Yvan Hope Cezar, 23 years old and currently residing at Compostela,


Davao de Oro. My parents' names are Alberto and Nenita Cezar. I am pursuing my
college degree in Business Administration major in Human Resource Management
at UM Tagum College.

When it comes to working in any environment, I am an outgoing, optimistic,


and welcoming young man who possesses a lot of perseverance and an open mind.
I enjoy learning new things whenever possible and I have a very caring personality.
All of my experiences have helped me improve aspects of myself, for which I am
grateful, and I am determined in facing all types of challenges while working on a
daily basis.

I am a Certified Human Resource Associate and I have my affiliations in


school organizations as an active student leader. I am preserved and committed to
all my endeavors and I always look for avenues where I can maximize my potential
and expand the reach of my capabilities.

BASIC INFORMATION OF HOST TRAINING ESTABLISHMENT

Tagum (formerly Municipality of Magugpo) was created by virtue of Executive


Order No. 452, issued by then Commonwealth President Manuel L. Roxas on June
23, 1941 by a Bill sponsored by Assemblyman Cesar Sotto, Davao’s representative
to the National Assembly.

The Municipality of Tagum was converted into a Component City known as


the “City of Tagum” by virtue of Republic No. 8472, approved on January 30, 1998
and ratified into law through a plebiscite on March 7, 1998.

Under such law, the City of Tagum constitutes a political body corporate and
as such is endowed with the attributes of perpetual succession and possessed of the
powers which pertain to a municipal corporation to be exercised in conformity with
the provisions charter.
At present, the City is classified under First Class Component City of which
certain independence in terms of revenue collection, salary compensation
classification and other financial and operational matters are being adopted and
exercised by the LGU.

improvement, adversity; you just got to take it all in and do what's right, continue to
grow, continue to live in the moment.”

Experience in my eyes is a valuable thing as I have to be brave to take


risks in life. It is not a fantasy I made up, but rather a reality I had to face. I was able
to discover myself from various perspectives by doing my internship in a real-world
setting. It guided me in controlling and developing my attitude and behavior as I dealt
with various people and situations.

 City Human Resource Management Office
 1. Assists the LCE in the operation, direction and management of the HRM
Office;
 2. Assists the LCE in the recruitment, selection, placement and development
 of officials and employees;
 3. Assists the LCE in the implementation and enforcement of civil service
 laws, rules and regulations and other pertinent laws and ordinances;
 4. Assists the LCE in the implementation of mandated salary standardization
and increases of employees’ benefits;
 5. Assists the LCE in monitoring administrative violations and in the imposition
of sanctions;
 6. Prepares annual budget for the HRMO operations;
 7. Formulates plans and recommends to the City Administrator for approval
 by the LCE, career and personnel development program for city officials and
employees;
 8. Formulates plans and recommends to the City Administrator for approval
 by the LCE, systems and standards of performance evaluation;
 9. Formulates plans and recommends to the City Administrator for approval
 by the LCE and for consideration by the Sangguniang Panlungsod,
 Employees Incentives, Fringe Benefits and Awards System;
 10. Prepares payroll of officials and employees;
 11. Coordinates with the City Administrator in the implementation of the
 personnel related development programs and activities;
 12. Coordinates with the City Department, Division and Section Heads on
 administrative and personneling issues under their supervision;
 13. Conducts seminars, workshops and information dissemination on
 personnel related issuances;
 14. Conducts periodic personnel audit and inventory;
 15. Conducts routinary personnel counseling and preliminary administrative
investigations;
 16. Prepares annual plantilla of personnel;
 17. Establishes liaisoning services with national offices and agencies: CSC,
GSIS, PhilHealth, PAG-IBIG, DILG, DBM, DOF and OMBUDSMAN;
 18. Establishes and maintains Personnel Records System and Management;
 19. Submits periodic personnel reports to concerned offices and agencies;
 20. Recommends to the LCE for approval, innovations and changes in the
 organizational structure;
 21. Performs such other functions and duties and exercises such other
powers as provided for under RA No. 7160, otherwise known as the Local
Government
 Code of 1991, and those that are prescribed by law or ordinance; and
 22. Submits Office Performance Commitment and Review (OPCR) and OPCR
 Target.

Summary of Duties and Responsibilities Performed

1. Assist in keeping and maintaining administrative records for an orderly and


effective filing, storage and retrieval system;
2. Assist in preparing draft of communications, reports and other related
documents;
3. Assist in the maintenance, updating and safeguarding of personnel
folder and personal records; and
4. Assist in performing other functions as may be assigned by the supervisors;
and
5. Assist in the submission of Individual Performance Commitment and
Review (IPCR) with Monthly Performance Output Report (MPOR) and IPCR Target
every
Semester.
6. Assists in the consolidation of reports from various offices for presentation
to the City Administrator for further recommendations to the LCE;
7. Deliver communications to other offices;
8. Assist in the preparation of documents to be signed by the City
Administrator;
9. Assist in providing various secretarial/clerical assistance, i.e., keeping
appointment schedules and handling of social schedule, arranging appearance or
substitution at official functions; acting as receptionist, taking phone calls, directing
walk-ins and handling inquiries; receiving incoming and serving outgoing
communications to concerned persons/ offices.
10. Assist in forwarding the outgoing communication outside the office’s
premises.
