This will be the landing page of the
Online Leave Application System
 Enter the Employee code

Note: Once you have entered your Employee Code the following
information will be loaded:
a.1 name of school
a.2 Name
a.3 Position
a.4 Email
a.5 Number of Remaining Service Credit
 Date of Filling
Enter the number of days to be applied Note: the date of filling must be
the date when your application
for leave must be submitted.

- Date covered of leave Select which type of leave
Choose File select the filled-out PDF E-Form6
Note: if you have an attachment like medical
certificate kindly compressed the file together with Double check the form, if all the
the other supporting documents into 1 file in a field are properly filled out then
.zip format click submit.
