(Sign & stamp).
3. RCM/ Site-In-Charge approval (Original). – To be submitted by OS Team
4. Undertaking to fulfil liabilities of contract labour to be engaged (original).

Undertaking
(Annexure-I)
We undertake to fulfil all our liabilities in respect of contract labour to be engaged by us
in Panipat Refinery as are laid down under various statutes and Rules framed there
under and are applicable or would be applicable to us.
We also undertake to fulfil the following obligation on our part:
1. To provide canteen facilities (if the contractor engaged 100 or more contract
labour)
2. To provide rest room, where contractor labour is required to stay at night in
connection with the work.
3. To provide wholesome drinking water, latrines & urinals and washing facilities at
convenient places.
4. To provide First Aid box in every place where contract labour is employed.
5. To pay minimum wages to various categories of contract labour as prescribed
from time to time.
6. To pay wages to each worker employed as contract labour in the presence of
representative of the principal employer and shall be certified by him.
7. To maintain register/ records as prescribed under the rules in respect of the
contract labour and furnish data to the principal employer.
8. To deposit the PF contribution in respect of each of contract labour and furnish a
copy of the documentary proof to this effect to the principal employer or his
representative as may be asked from time to time.
9. To pay ESI dues wherever applicable in respect of each of his contract labour.
10.To take comprehensive policy of contract labour for the payment of
compensation under Employees Compensation Act, 1923 in case of accident
resulting in injuries or death (This would not be applicable where contractor is
covered under the ESI).
11. To take Third Party Insurance (Public Liability Insurance).
