APPLICATION FOR LETTERS OF ADMINISTRATION

I, ATHIENO LOY AND OBISO FREDRICK from Bira North Village, Lwala Parish,
Mulanda Sub-county, Tororo District do hereby apply to this honourable court for a Grant of
Letters of Administration to the estate of the late NO. UA016373-WO2 OFUMBI WILSON
OCHIENG, who died on the 8th day of January, 2016.
The deceased is survived by:-
1) Athieno Loy (widow) 2) Nyadoi Hariet- 46 years(daughter) 3) Adongo Beatrice- 38
years(daughter)
The deceased left:
1. 2 pieces of land (4 acres) at Bira North Village, Mulanda Sub county, Tororo District.
2. One permanent house.
3. Gratuity and Pension.
4. An account no. 9520009841 with Centenary Bank, Tororo branch.

The said NO. UA016373-WO2 OFUMBI WILSON OCHIENG at the time of his death had a
fixed place of abode at Bira North Village, Lwala Parish Mulanda Sub-county, Tororo District
within the local limits of Jurisdiction of this Honourable court.

This application is made by we as the widow and grandson to the deceased and I believe that the
value of the said estate is likely to be Shs. 18,000,000/= (Eighteen million shillings)

And I, ATHIENO LOY AND OBISO FREDRICK the applicant do solemnly and sincerely
declare that what is stated in this application is true to the best of my own knowledge and belief
and I make this solemnly declaration conscientiously believing the same to be true and by
virtue of the Provisions of the Statutory Declaration Act, 2000.

Sub-county, Tororo District hereby solemnly and sincerely declare that NO. UA016373-WO2
OFUMBI WILSON OCHIENG of Bira Noth Village, Lwala Parish, Mulanda Sub-county,
Tororo District died intestate on the 8 th day of January, 2016 and that we the widow and
grandson to the said deceased and that I shall well and faithfully administer the estate and
effects according to the law and that I shall exhibit a true and perfect inventory of all and
singular off the said estate and effects and render a just and true account thereof whenever
required by law so to do and that the said deceased died of illness.
