As a parent/guardian of the above-named child participating in the Berkeley Math

Tournament (BMT) 2023, I understand that this Agreement on gaining sponsorship from
SIMCC form must be signed by me in order for my child to participate in the BMT on 26
November 2023.

For purposes of this agreement, my child has obtained scholarship for Global Math Olympiad
Scholars (GMOS) for Top 5 contestants in AMO 2022 from grades 8 to 12 (worth RM400). I
confirm and agree that my child will take part in the BMT. I undertake that if my child fail to
attend the BMT, I shall pay Penalty cost RM 500.
