UNDERTAKING TO APPEAR AND

WAIVER OF APPEARANCE

Philippine Currency for each case for my temporary release. As such, I hereby undertake to fulfill
the following conditions of my bail, thus:

I shall appear before this Honorable Court whenever required in accordance with Section
2 Rule 114 of the Revised Rules of Criminal procedure;

I shall at all times hold myself amenable with the orders and processes of this Honorable
Court;

In case of I jump bail or fail to appear at the trial despite due notice, my absence shall be
deemed an EXPRESS WAIVER of my right to be present and in which case, trial may proceed in
my absence without prejudice to the issuance of a warrant of arrest;

It is understood and I agree that any reference to the person of the accused mentioned in
its information/complaint shall mean to be referring to my person and that I am the same person
being named in the information/complaint;

I shall notify the court of any change in my address and shall not leave the country except
upon prior authorization from the Court;

I shall surrender myself to the court for execution of the final judgment.

In order to warrant that I am a resident of the above-stated residence address. A sketch

of my residence is attached herewith.
