a. Annual Permit to Operate from Philippine Ports Authority X X X
b. PRC Certificate and ID; and BOC Accreditation X
c. Authority to Operate from the Civil Aeronautics Board (for Air Freight) X
d. Certificate of Accreditation from the DTI-FTEB (Sea Freight) X
e. List of Vehicles with attached valid LTFRB Case Decision & updated OR/CR X
f. List of drivers/helpers certified by highest official X
g. Surety Bond (can also be submitted together with the payment of franchise fee)

Once approved by the Director General of PEZA, prior to the release of the Authority, please submit the following:
