FIRST LEVEL OF ASSESSMENT

WELLNESS

 The family has been staying in the community for about 15 years now. The family seldom participates
in activities in the community such as community assemblies and other social activities that are being
held within the barangay.
 Family observes proper methods of segregating and recycling of garbage.
 One of the family’s sources of information from the barangay is from the barangay kagawad, barangay
health worker, and through a public information system available in the community. Their means of
transportation going to the city is through solar jeep, multicabs, and air-conditioned buses. They have
regular and stable means of transportation within the area of the barangay.

Spiritual Well being:


Family members are Roman Catholic. They pray the Holy Rosary every 6PM. They offer a prayer before
sleeping. The family regularly hears mass every Sunday at the nearby chapel.

READINESS

 Mrs. Light has complete immunizations and does regular prenatal visits.
 Mr. Light is ready to stop his drinking habits. As verbalized, “Andam na ako pag-undang hini nga akon
pag-inom hin tuba kada semana”.
 Bright Light is currently at 6 weeks postpartum. She is compliant with the post-natal check-up and
scheduled immunizations of Neon Light. She also encountered no problems with breastfeeding Neon
Light.
 Celestial light uses herbal plants available in their garden for her rash.
 Neon Light is compliant with her immunization and is exclusively breastfeeding.
 The family’s diet consists of fish and vegetables and they eat 3 times a day.
 The family uses sleeping mats and mosquito nets to protect themselves. They also use slippers in their
house.

HOME AND ENVIRONMENT

HEALTH THREATS

consumption.

HEALTH DEFICIT

•Dark Knight (was) recently experienced weight loss, abdominal pain, and identified a worm in his stool after
defecating, which prompted him to seek consultation at the district health center. He was advised for a direct
fecal smear. Results showed positive for Ancylostoma dudenale.

Definition of Anclyostoma duodenale:

-Infects a person through the oral route (known as Wakana syndrome. It is a species of the roundworm genus
Ancylostoma. It is a parasitic nematode worm and commonly known as the Old World hookworm. It lives in the
small intestine of hosts such as humans, cats and dogs, where it is able to mate and mature.

•Celestial Light is a 16-year-old, grade 9 student at Scandinavian National High School. She currently has a
pimple like rash present across most areas of her body, experiencing severe itching and thickening of her skin.
She described the itch as worse at night. She uses the herbal plants that are available in their garden.

CELESTIAL LIGHT Summary:

•Moon Light is a 10-year-old, grade 4 student at San Roque elementary School. He is currently experiencing
the same signs and symptoms as Celestial Light.

MOON LIGHT Summary:

-Severe itching and Skin thickening (mostly at night)

•Star Light is 4 years old, currently a Kindergarten 1 student at San Roque Elementary School. She

is 101 centimeters tall and weighs 10.9 kilograms upon assessment at their district health center. She

currently has fever, maculopapular rashes, Koplik spots, and her eyes are sensitive to light. She was also
recently advised for direct fecal smear but results showed no infection.
STAR LIGHT Summary:

-Underweight (BMI: 10.27)

-Negative Results of Fecal Smear

(Justification: Failure to thrive/devlop according to normal rate given as Star light is Underweight with BMI of

 Dark Knight is the husband of Bright Light. He lives with the Light Family after Bright Light gave birth to
Neon Light.

studying after she gave birth to Neon Light. She is currently at 6 weeks postpartum. She is compliant
