Pursuant to COMELEC Resolution No. 10211 (as amended), by unanimous vote of the
undersigned Electoral Board, for their protection or for the protection of the election documents
and paraphernalia:

[ ] Please direct an officer to remove and/or take custody of _____________________

[ ] who refuses to obey our lawful order

[ ] conducts himself/herself in a disorderly manner thereby interrupting or
disturbing the proceedings.

[ ] Order the detail of sufficient number of PNP personnel to stay outside our polling
precinct within a radius of 30 meters or near enough to be easily called by the
Electoral Board at any time.
