Ltd. in 2003 but has not withdrawn his provident fund dues. 3.     solemnly affirm that :

    1996 to 01-March-2003 and I was a member of Provident
    Fund Bearing A/c No. MH/29433/101 (DAIRY FARM
    EQUIPMENT)

    2. I resigned from the services with effect from 01-March-


    2003.

    3. I have not withdrawn my P F dues to me till today.

    4. I have opened my Bank A/c No. 003901009947 on 01-Apr-


    1998 and still it is not closed.

    5. I indemnify to the central board to trustee, employees


    provident fund organization against any loss and wrong
    payment which may be sustained by payment of PF & E P
    F dues to me.
