    Most respectfully, I, Mst. Fatima W/o Muhammad Bachal,


    Muslim, Adult, Resident of House B-2/46, Street No. 9, Rehman Colony,
    Manzoor Colony, Karachi, bearing CNIC No. 42301-9328803-2, submit
    to your kind authority that when I went to NADRA Mega Centre, DHA
    Phase- I for the purpose to prepare a B-Form for my Children, but when I
    provided my fingers impressions for the verification the fingers
    impressions were not visible/clear/matched, so the NADRA Officials
    verbally said that I need a medical certificate for such purpose, therefore I
    `most respectfully, begs to your kind authority to kindly, issue a
    certificate for the verification of my fingers impressions.

    are annexed for the perusal of your kind authority}
