CERTIFICATION

First, you need to secure your schedule of appointment through online, kindly visit this link
https://online.prc.gov.ph/ , create/register your account (or sign in if you already have an account) and
select your transaction. Further, please ensure that said transaction is already paid before proceeding to
our office on your chosen schedule of appointment.
Provided herein are the requirements and procedures as to transacting for issuance of various
certifications:

Request for the Following:


 Certificate of Good Standing
 Original Board Rating
 Original Board Passing
Requirements:
 Online Appointment Schedule (make sure the transaction is already PAID)
 Certification Request Form (downloadable in your PRC Account)
 Updated valid PRC License Card (for registered professionals)
 Updated valid ID (unregistered passers)
Note: Provide documentary stamps (2 pieces per copy)

Process: Per Appointment Schedule

REQUIREMENTS FOR REPRESENTATIVE (CERTIFICATION):


If representative is PRC Licensed Card holder :
 Authorization Letter
 Valid PRC License Card (representative)
Note: Representative should bring the ORIGINAL documents required for the issuance of requested
certifications.
If representative is NOT PRC Licensed Card holder :
 Special Power of Attorney
 One (1) Valid ID (indicating the representative’s name and date of birth)
Note: Representative should bring the ORIGINAL documents required for the issuance of requested
certifications.
