upcoming Intersection Sports Fest 2022 to be held on May 30 – June 3, 2022 at Green Fields
Integrated School of Laguna Inc.

I hereby confirm that I am not having any objection related to the participation of my child to the
said activity.

I, as a parent and/or legal guardian, remain legally responsible for any personal actions taken by
the above-named participant.
