If you are currently under 18 years of age at the time of contract submission, you must complete this
form, including signatures from yourself and your parent/guardian, before uploading it in the housing
application when prompted. If this form is missing, your application will be considered incomplete.

furnished is true and complete. I understand and agree to the terms and conditions stated in the
Housing & Dining Residence Hall Contract, 2020-21.

I give permission to the University of Colorado to authorize emergency transportation as per the policy
outlined in the Personal Injury & Illness section of the Residential Handbook.

I hereby understand the conditions for assigning accommodations as stated in the contract. By signing
