The following fields must be completed for each Beneficial Owner:
Forename – The forename of the beneficial owner must be provided in full.
Surname – The surname of the beneficial owner must be provided in full.
Date of Birth – The date of birth of the beneficial owner must be provided in the format of dd/mm/yyyy.
PPSN Yes/No – Select Yes if the beneficial owner holds a Personal Public Service Number (PPSN), otherwise select No.
