    Nearest person or discoverer

    Notify victims’ next of kin


    Alert supervisor and ERT Leader. Stop
    work, call hospital and police
    immediately
    Lodge report at police station
    Inform Facility Manager/ PP/ Project
    Owner Assess surroundings for any further
    and DOSH immediately threat or danger
    Submit preliminary report within 24
    Visit vicinity to gather relevant information
    hours

    Secure or cordon vicinity

    Recommend immediate temporary preventive measure


    Establish investigation team to
    conduct

    Obtain police approval for the body to be sent to the


    nearest hospital

    Prepare investigation report and


    submit to DOSH as soon as and
    c.c. a copy of the form to PP and
    Project Owner within 7 days Obtain death certification from hospital & burial permit from police.
