JOINT AFFIDAVIT OF LEGITIMATION

We, SAMUEL RABONG ARBELO and ANTONIA FLORENDO ORTULA, both of

legal age, Filipinos, and residents of Golden Valley, Brgy. Sicsican, Puerto Princesa
City, Palawan, after having been duly sworn in accordance with law, do hereby depose
and say:

1. Without the benefit of marriage there being no legal impediment, we were

blessed with child named, KIMBERLY JOY ORTULA ARBELO, born on
August 25, 1993 at Puerto Princesa City, Palawan;

2. That I ANTONIA FLORENDO ORTULA was only 17 years old when I gave
birth to KIMBERLY JOY ORTULA ARBELO;

3. That we are executing this affidavit to attest to the truthfulness of the

foregoing statements and to be able to legitimize the birth of our child,
KIMBERLY JOY ORTULA ARBELO with the Office of the Civil Registrar of
Puerto Princesa City and other offices, and for whatever legal purpose the
same may serve.
