    CERTIFICATION

    This serves as confirmation that MRS. EDELTRUDES CABANIT SABUERO is a


    member to Saint Michael Senior Citizen Chapter. She had just recently registered
    with the National Senior Citizen Commission, but sadly, she passed away a few
    days prior to the payout date. Since Mrs. Sabuero is an indigent widowed woman
    and lived alone by herself, it is fair and appropriate for her nearest relative to
    make a claim on her behalf.

    This affirms further that she is using the OSCA I.D. number 18-31429 from
    her application form, which is attached to this document, to support her
    membership in place of her missing OSCA I.D. card.

    I'm hoping her benefits will be paid out so they may be utilized for her
    funeral.
