CERTIFICATE OF INDIGENCY

This is to certify that Ms. Olivia Edejer born on September 26, 1963, and presently residing
at Purok Kamote Barangay Malabon, Municipality of Candelaria, Province of Zambales, is
recognized by the Municipal Social Welfare and Development Office (MSWDO) as an indigent
person.
The aforementioned individual, having been subjected to a thorough assessment conducted by
the MSWDO, has been determined to be in a state of economic hardship, with an evident inability to
meet the basic living expenses necessary for a decent standard of living.
The assessment took into consideration various factors, including but not limited to income,
employment status, family size, and living conditions. Based on the findings, it has been established
that Ms. Olivia Edejer is in need of financial assistance and support for her corrected Name.

This CERTIFICATE OF INDIGENCY is hereby issued for the purpose of enabling Ms.
Olivia Edejer to avail of benefits, privileges, and services provided by the government and other
institutions aimed at supporting indigent individuals within the jurisdiction of the MSWDO of
Candelaria, Zambales. Issued on this 25th day of January 25, 2024.
