PART II Narrative Report

1. Rationale

In line with the Department Order Number 39, s. 2017 Operational Guidelines on the
Implementation of School-Based Feeding Program for School Years 2017-2022. Name of School
supported and conducted an Orientation for the Parents Recipients of the School-Based Feeding
Program (SBFP).

2. Narration of Accomplishments

The Parents’ Beneficiaries were all called last July 25, 2019 at 8:00 in the morning by the
SBFP Coordinator _______ and School Health Coordinator ________.

The orientation started with a prayer led by ________, followed by the singing of National
Anthem conducted by ______.

School-Based Feeding Program Coordinator gave her message and some insights regarding
on this activity. They were informed of their Nutritional Status and the programs objectives and
purposes which they would take part. _________, ensured the 100% deworming target for the
Beneficiaries of Feeding Program. She also emphasized those who were severely wasted and wasted
pupils from Kinder to Grade six levels. During Feeding, health care practices such as handwashing
and tooth brushing are also promoted.

After the orientation, the parents were group for their daily schedule. ______ and the School
Head made a daily schedule of the parents who will be cooking for the recipients
