CONSENT OF PARENT/GUARDIAN

I, Elmer P. Estrella, of legal age, residing at Malis, Guiguinto, Bulacan, and

parent/guardian of Erica Joy S. Estrella, student in the College of Science, Bulacan State
University, Main Campus, do hereby authorize him/her to attend the face-to-face internship,
outside the Bulacan State University at Manulife from June 13, 2023 to July 25, 2023. I
understand that he/she will be guided by the University rules on student conduct, rules and
regulations.

My child will comply with the laws and regulations to be served by the Host Training
Establishment for his/her good and safety.

I agree that I have a responsibility, Elmer P. Estrella, Bulacan State University, the
Manulife and other deputies if in case of an incident with my child during his/her face-to-face
internship.

We hereby declare that we have read and understood the contents of this document
prior to signing thereof and that this is made freely and voluntarily and with full knowledge
of our rights under the laws of the Philippines.
