COLLEGE STUDENTS AND PARENT/S OR PARENTS-SUBSTITUTES
CONSENT FORM
About the Program

Tara, BASA! Tutoring Program is a social welfare and development model of


intervention wherein college students shall be capacitated and deployed as Learning
Facilitators (LFs) who will teach non/struggling readers how to read or as Youth
Development Workers (YDWs) who will conduct sessions with parents or parents-
substitute on care and protection of children and guide them to be “Nanay-Tatay
Teacher.”

The program will be pilot tested in Public Elementary Schools from the 16 cities and 1
municipality in the National Capital Region (NCR).

This Consent is voluntarily given by the college students who shall be engaged in the
implementation of Tara, Basa! Tutoring Program and their parents or parent-substitute
as well.

Roles and Functions of SLUCs

● Screen and endorse the list of College Students as applying as Learning


Facilitators and Youth Development Workers by SLUC to DSWD Social
Technology Bureau.
● Identify venues for the capacity building of Learning Facilitators (LF) and/or Youth
Development Workers (YDW) within the school.
● Participate in Project Meetings related to monitoring,
assessment, and evaluation.
● Monitor the Learning Facilitators and the Youth Development Workers during the
20-day session

Roles and Responsibilities of Learning Facilitators or Youth Development


Workers

1) Learning Facilitator (LFs)

a) Attend the capacity building on facilitating the reading fluency


non/struggling readers how to read;
b) Conduct learning sessions for non/struggling readers for a maximum of
twenty (20) days following the prescribed learning modules ;
c) Adhere to the Cash for Work Contract stipulating their roles and
responsibilities in the project;
d) Submit the Attendance Sheets of assigned non/struggling readers every
learning sessions; and
e) Submit the Daily Time Record and Certificate of Appearance signed by
reading coordinator from the Public Elementary School of assignment.

2) Youth Development Workers (YDWs)

a) Attend capacity building on facilitating the Nanay-Tatay Teacher Sessions;


b) Conduct the 20-days parenting enhancement sessions with parents/
parent-substitutes using the prescribed Nanay-Tatay Teacher Modules;
c) Adhere to the Cash for Work Contract stipulating their roles and
responsibilities in the project;
d) Submit the Daily Time Record and Certificate of Appearance signed by
reading coordinator from the Public Elementary School of assignment;
e) Submit Monitoring Report to the DSWD Field Office NCR; and

f) Collaborate with 4Ps Family Development Session (FDS) facilitators and
Local Social Welfare and Development Officers (LSWDOs) for the conduct
of Nanay-Tatay Teacher Sessions.

Benefits to the Learning Facilitators or Youth Development Workers

The DSWD shall provide the College Students who shall be deployed as LFs or YDWs
cash incentives equivalent to 100% of the Regional Daily Minimum Wage for a period of
20 days, amounting to a total of P11,400.00 (tax free), following the existing government
rules and regulations.

Data Privacy and Confidentiality Clause

By signing this Consent Form, I/we grant my/our voluntary and informed consent to the
collection and processing of all Personal Data relating to me and my child. This was
freely disclosed to the DSWD Social Technology Bureau through this form in
accordance with Republic Act (R.A.) 10173, otherwise known as the “Data Privacy Act
of 2012” of the Republic of the Philippines, including its Implementing Rules and
Regulations (IRR) as well as all other guidelines and issuances by the National Privacy
Commission (NPC).
