I, EVELYN GUNDAY REYES, 32 years old, born on January 16, 1986, married, and a resident of ?

after having been sworn to in accordance with the law, do hereby depose and state:

That PMSg Jesiree Dornagon is the police officer who conducted and supervise my examination
as a witness at the SAMPALOC POLICE STATION, Sampaloc Manila;

That I am answering the questions herein fully conscious that I do so under oath and that I may
be criminally liable for false testimony or perjury;

The following are the questions propounded by PMSg Jesiree Dornagon and my answers
thereto in English language;

Questions:

POLICE: Do you swear to tell the truth and nothing but the truth?

EVELYN REYES: Yes Sir.

Police Officer:Do you know the reason why you are here?

EVELYN REYES: Yes Sir.

Police Officer: Are you same EVELYN REYES, the accused in this case for illegal recruitment and
industrial fraud now being investigated by this office?

EVELYN REYES: Yes Ma'am.

Police Officer: Ms. Reyes, can you confirm your identity for the record?

Evelyn Reyes: Yes, I am Evelyn Reyes.

Police Officer: Ms. Reyes, you are being interviewed as part of an investigation into allegations
of illegal recruitment and industrial fraud. Are you aware of the nature of these allegations?

Evelyn Reyes: Yes, I am aware.

Police Officer: Did you, Evelyn Reyes, engage in any illegal recruitment activities, as alleged?

Evelyn Reyes: No, I did not.

Police Officer: Ms. Reyes, there are statements from individuals claiming that they were
promised employment under your representation but were deceived. What is your response to
these allegations?

Evelyn Reyes: I deny deceiving anyone. My interactions were legitimate, and I did not engage in
fraudulent activities.

Police Officer: Are you affiliated with any recruitment agency or organization involved in
industrial fraud?

Evelyn Reyes: No, I am not associated with any fraudulent activities or organizations. I
conducted my business within the bounds of the law.

Police Officer: Ms. Reyes, during the investigation, documents were obtained suggesting
financial transactions related to the alleged illegal recruitment. Can you explain these
transactions?

Evelyn Reyes: I am willing to provide records and explanations for any financial transactions.
They were conducted within legal parameters.
Police Officer: Have you coerced or forced anyone into any employment agreements against
their will?

Evelyn Reyes: No, I have not coerced or forced anyone. All engagements were voluntary.

Police Officer: Ms. Reyes, are you willing to cooperate fully with the ongoing investigation,
including providing any necessary documents and information?

Evelyn Reyes: Yes, I am willing to cooperate fully and provide any necessary documents or
information to prove my innocence.

Police Officr: Do you have anything else to say, Ms. Reyes

Evelyn Reyes: No more Ma'am.
