other regulatory requirements. I claim responsible to complete all my requirements on or before

Pre-Employment Requirements Checklist

1. Updated Resume with photo

2. 2 pcs. 2x2 and 2 pcs. 1x1 most recent ID picture in white background

3. Original copy of Barangay Clearance

4. Original copy of NBI Clearance

5. Photocopy of the SSS, TIN, HDMF, and PhilHealth numbers. Documents or IDs
must be from the government agencies

6. Photocopy of PSA Birth Certificate

 7. Photocopy of PSA Marriage Contract (if applicable)

8. Photocopy of Transcript of Records (TOR) and Diploma

9. Photocopy of Professional License (if applicable)

10. Photocopy of Certificate of Employment

11. Photocopy of BIR 2316 (For those with previous employer)

and Drug Test with Medical Certificate of fit to work.
