Type of Structure : RENOVATION OF TWO STOREY RESIDENCE

This manifestation is to certify that the undersigned had checked and inspected the existing two
storey residence on November 16, 2023.

Upon inspecting, investigating, and studying the site condition, the existing structural members for column
footings, columns, beams, and slab have found that the above mentioned subject structure are adequate
and is safe and can withstand to add third floor.

The subject structure had complied in accordance with the requirements on the provision of the National
Building Code of the Philippines (PD 1096) as well as the NSCP 2015.

I therefore certify that the subject structure is safe and sound and structurally stable and pose no danger
to both life and property. I do also certify that the said structure is safe for use and human habitation.

This certification is being issued in response to owner’s request.
